While medical community is tremendously working on testing already developed vaccines and developing new ones, scientists test existing methods of traditional and alternative medicine.
Lopinavir and ritonavir are well-known antivirals used for combating HIV. There is a strong scientific evidence that they may be helpful for treatment of new coronavirus (see below).
Generic Kaletra by famous Indian company Cipla Limited contains both ingredients in 4:1 ratio (lopinavir 200 mg and ritonavir 50 mg). It can also be dispensed under brand name Lopimune.
State bodies of China recommended its use for treating the novel epidemic disease. Some Chinese patients with HIV who receive Kaletra but for some reason don’t take it, have decided to create a platform to distribute unused meds to coronavirus sufferers.
Amidst loud news about Kaletra, Russian company R-Pharm announced plans to increase production of this medication to meet the increasing need for it in the pharmaceutical market.
An American pharma giant AbbVie Inc. which also produces the product based on these two components donated a supply of remedies to the Chinese health authorities.
When it comes to human immunodeficiency virus therapy, lopinavir acts as a suppressor of HIV-1 and HIV-2 proteases. This prevents viral proteins from synthesis and leads to formation of immature and nonfunctioning virus.
Ritonavir is also an HIV-1 and HIV-2 protease inhibitor, an active peptidomimetic that simulates the functionality of peptides. Inhibition of HIV proteases also leads to the formation of a virus that is not capable of infection. Ritonavir inhibits CYP3A isoenzyme-mediated metabolism of lopinavir in the liver, as a result of which the effectiveness of the second component of the medication increases.
But the medicine cannot kill the virus completely, this is not a cure for HIV because as this is an incurable disease. However, in case of coronavirus Kaletra administration may lead to full recovery.
Scientists believe that Kaletra affects intracellular processes of coronavirus replication thereby inhibiting its reproduction.
Generic Kaletra may be administered for coronavirus disease (COVID-19 caused by SARS-CoV-2) treatment solely or as a part of combination therapy, for instance, along with reverse transcriptase inhibitors. In the context of SARS epidemy (caused by SARS-CoV), there were studies confirming preventive effect of the medication against SARS-CoV. In this regard, it should be noted that SARS-CoV-2 is about 70% similar to SARS-CoV.
So, Kaletra may be potentially used as preventive agent to protect from new coronavirus.
However, clinical studies are needed to confirm whether it is true.
The WHO has been noted that the abovementioned pharmaceutical combination could provide clinical benefits.
As reported by Clinical Trials Arena, Lopinavir plus ritonavir in conjunction with ribavirin showed decreased fatality rate and milder illness course during clinical trials in individuals in the 2003 SARS outbreak.
HIV patients should take 2 pills two times a day or 4 pills once a day. As for coronavirus, China’s National Health Commission issued a special guideline on January 27 (2020), according to which patients need to take the medication twice a day, 2 pills at a time.
The regulator didn’t initially stipulated how long it should be taken, but on February 19 it updated the statement, indicating that the therapy should not last longer than 10 days.
So current guideline is as follows: 2 pills a day for up to 10 days.
The drug is contraindicated in the following cases:
- increased sensitivity to ritonavir or lopinavir,
- intake of medications which are primarily metabolized via CYP3A mechanism (like Viagra, Cialis, and Amiodarone),
-intake of potent CYP3A inducers which can significantly reduce lopinavir plasma, -concentrations (e.g. Alfuzosin, Dronedarone),
-acute liver failure,
-consumption of St. John’s wort supplements.
Caution is required when using the drug in patients with pancreatitis, hemophilia, cirrhosis, viral hepatitis B and C. The elderly should also be cautious.
In some patients, adverse events may occur. They include but not limited to headache, dizziness, elevated blood sugar levels, muscle weakness, skin reactions, appetite loss, itching, nausea, and vomiting.
