Common use

Contrave is a combination medication primarily used as part of a weight management plan in adults with obesity or overweight conditions, especially those with weight-related health issues like hypertension or type 2 diabetes. Bupropion is an atypical antidepressant that affects dopamine and norepinephrine levels, while naltrexone is an opioid antagonist that also affects brain pathways involved in cravings and reward. Together, they help reduce appetite and control food cravings. This combination is typically used alongside a reduced-calorie diet and increased physical activity.

Dosage and direction

The medication is taken orally, usually starting with a low dose that is gradually increased over several weeks to minimize side effects. It is recommended to take the tablets twice daily, with or without food, but not with high-fat meals due to increased risk of seizures. Patients should follow the titration schedule provided by their healthcare provider. Do not crush, chew, or split the tablets.

Precautions

Before using this medication, inform your doctor if you have a history of seizures, eating disorders, uncontrolled hypertension, or opioid dependence. Regular monitoring of blood pressure and mental health is advised, as bupropion may increase the risk of suicidal thoughts in some individuals. Naltrexone can block the effects of opioid pain medications, so it must not be used by patients currently taking opioids. Avoid alcohol and high-fat meals during treatment.

Contraindications

Contrave is contraindicated in individuals with uncontrolled hypertension, seizure disorders, bulimia, anorexia nervosa, or those undergoing abrupt withdrawal from alcohol, benzodiazepines, or barbiturates. It must not be used in patients using opioids or undergoing opioid withdrawal. The combination is also contraindicated in those with a known hypersensitivity to either component. Use during pregnancy is not recommended unless clearly needed.

Possible side effect

Common side effects include nausea, constipation, headache, dizziness, dry mouth, and insomnia. Some individuals may experience increased blood pressure or heart rate. Rare but serious side effects include seizures, suicidal thoughts, liver damage, or severe allergic reactions. Report any unusual or severe symptoms to a healthcare provider immediately.

Drug interaction

This medication may interact with MAO inhibitors, opioids, antidepressants, and certain antipsychotics. Combining it with drugs that lower the seizure threshold or that affect the central nervous system can increase the risk of adverse effects. Alcohol may enhance the risk of side effects and should be avoided. Always inform your doctor about all medications and supplements you are taking.

Missed dose

If you miss a dose, take it as soon as you remember unless it is close to the time of your next dose. Do not take two doses at once to make up for a missed one. Missing doses may reduce the effectiveness of treatment. Try to take the medication at the same times each day to maintain consistency.
